4. Health statement: This program is intense and demanding, which presupposes some physical stamina and emotional balance. If you are currently under the care of a physician or if you have a condition of which we should be informed, please provide details
5. A brief autobiography (5-10 pp.) from the standpoint of your relationship to your voice;
   A statement about what attracts you to this work and what hopes and fears you may have with regard to engaging in it
6. A non-refundable application fee of 25 USD. Pay through PayPal or in a
   money order in USD payable to The Norma G. Canner Foundation.
